I'm needing to create a online survey with just 5 or 6 questions and store
the results in an Excel spreadsheet. Is there any documentation on how to
do this?

I suggest using existing services, like http://www.surveymonkey.com, for the
web-based survey.

Then download the results and analyze using Excel.
