How can I change labels to a table for showing menu choices

I have labels w/names/addresses and I want to re format them into a table for
showing dinner menu choices.
I am doing this in long, slow, and inefficient manner now?

Any good ideas?
thanks

Currently have three columns of labels, centered, w/name, address, and
city/state.
These people are responding to attending dinner, w/3 menu choices: chicken,
salmon, and tortellini. I want to translate each name column into three menu
choices so I can give to restaurant beforehand and perhaps, set up some type
of name/menu choice for a table setting at the restaurant. I am getting
something but v. messy now. Not clever enuf to write a "program" to do this.
