How to setup
 

I am trying to setup a spreadsheet to capture information regarding job exit
interviews. I want to rate each answer with a numeric value so I can chart
the information on a quarterly basis. I'm just not sure how to go about
setting this up. Does anyone have any ideas or a template they can refer me
to ? Any help would be greatly appreciated.
