Default Probability of Time Loss, Non-time Loss and Fatality

I am trying to design a solution to determine/analize the probabilities of Time Loss unsafeevents, Non-time Loss unsafeand Fatality events. The sample (actual) data is the following. I will appreciate your help.

Year Events Unsafe Events Time-Loss Non-Time Loss Fatality
Yr1 8,082 3 0 1 0
Yr2 9,000 2 0 0 0
Yr3 10,766 11 0 0 0
Yr4 11,533 8 0 0 0
Total 39,381 24 0 1 0
